Description:

*Coordinates Valve and Structural Heart Disease referred patient schedule with clinic staff
and supervising physicians;
* Assures post-TAVR one-month and one-year follow-up yearly appointments are scheduled and
communicated effectively
* Assures compilation and availability of all necessary clinical data in preparation for
patient visit to clinic;
* Assures compilation and availability of all necessary clinical data in preparation for
inpatient TAVR or other structural heart procedure
* Directly communicates with referring physicians on patient s status in respect to current
and anticipated continuum of diagnostic work-up;
* Assures all patient charges are appropriately entered into the hospital accounting system
* Assures all patient clinical data is appropriately entered into the medical record
* Efficiently utilizes clinic resources to safely provide excellent patient care in a
cost-conscious manner;
* Works collaboratively with and assists other Cardiovascular ARNP Coordinators as
appropriate in providing seamless patient care
