General Summary

Responsible for the individual and aggregate/system community health agenda(s) of the University of Maryland Medical System (UMMS) and its Member Organizations. Oversees community health programs including community health policies, needs assessments, and adherence to regulatory and reporting requirements. Works collaboratively with cross-functional teams to coordinate all Population Health activities regarding interventions, evaluations, and management of specific health programs,

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

  1. Assesses the health needs of the communities served by UMMS affiliates through collaboration with community, government, and other agencies to conduct formal needs assessments focusing on populations that have been historically marginalized and underserved by the healthcare system.
  2. Reviews current local and national trends related to public health, community health improvement, and emerging health issues for program planning and communication to the community.
  3. Sets goals and strategies that will assist in managing the development and success of the department.
  4. Works in collaboration with UMMS affiliates to develop community health priorities and implementation plans locally and for the System.
  5. Plans, develops, and evaluates appropriate community health programs based on the identified community needs.
  6. Works collaboratively to implement the portfolio of population health projects and initiatives to meet the Maryland Total Cost of Care model for individuals, to better the health of specific populations, and to drive health equity in Maryland.
  7. Collaborates with UMMS Member Organizations to cascade system priorities and objectives at a local level.
  8. Represents UMMS in city-wide, county-wide, and state-wide community health committees, task forces, and workgroups to collaborate on the development and implementation of population health and health equity initiatives.
  9. Measures the effectiveness of community benefits programs designed to improve the health status of focus communities served.
  10. Manages the accounting and reporting of community benefit activities and collaborates with departments and functions that are required to complete state and federal reporting of community benefit activities.
  11. Assures UMMS is in compliance with state and federal regulations related to community benefits programming for its affiliates, including completion of Community Health Needs Assessments and Community Health Improvement Plans.
  12. Prepares an integrated annual, system-wide community benefit report and communicates activities to key internal audiences and external constituencies.
  13. Identifies and pursues major grant opportunities with local hospitals/hospital systems and community-based organizations to support major population health improvement initiatives for the benefit of UMMS affiliates.

Requirements

Education and Experience

1. Master’s degree in nursing, public health, or health care administration required.

2. Ten (10) years’ experience in community health improvement or population health work within a large health system or local/state health department to include:

  • Conducting community-based health planning and programming with community health needs assessment experience.
  • Community-based health program planning and population health interventions.

3. Preferred experience with strategic plan development and implementation as well as organizational strategy integration.

Knowledge, Skills and Abilities

  1. Knowledge of state and federal requirements for community benefit reporting.
  2. Excellent interpersonal skills, with the ability to develop effective and collaborative working relationships with individuals representing a broad range of constituencies.
  3. Ability to manage multiple projects simultaneously. Excellent organizational and group facilitation skills.
  4. Excellent written and oral communication skills. Experience with Word, Excel, and PowerPoint, required. Experience with Access and Lyon software, preferred.

Required Physical and Environmental Demands

  1. Position works primarily in a standard office environment.
  2. Travel across Maryland may be required.
