Humana Healthy Horizons in Virginia is seeking Associate Director, Care Management (Care Management Extender Leader) who will direct and implement care management support in the areas of community resources, learning and development, process improvement, and care management audit in support of our Medicaid members in the commonwealth of Virginia. Their team will provide clinical and non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and well-being of members. The Associate Director, Care Management (Care Management Extender Leader) requires a solid understanding of how organization capabilities interrelate across department(s).
