Under the direction of SERDC Director of Health, as part of the integrated, multidisciplinary team, the Nurse Practitioner (NP) will integrate their in-depth knowledge of advanced nursing practice and theory, health management, health promotion, disease/injury prevention, and other relevant biomedical and psychosocial theories to provide comprehensive health services. The Nurse practitioner will work in collaboration with their clients and other health-care providers in the provision of high-quality patient-centered care. The NP works with diverse client populations in a variety of contexts and practice settings.

Duties and Responsibilities

  • Provide comprehensive health assessment(s), to diagnose health/illness conditions, and to treat and manage acute and chronic illness within a holistic model of care.
  • Order and interpret screening and diagnostic tests, perform procedures and prescribe medications, while integrating the principles of resource allocation and cost-effectiveness, in accordance with federal, provincial and territorial legislation and policy.
  • Applies current/relevant nursing and medical theory, which allows for contribution to knowledge, development, and involvement in advancing the nursing profession.
  • Advanced nursing care knowledge related to health assessment, diagnostics, pharmacotherapeutics and invasive and minor surgical procedures.
  • Incorporates medical knowledge and advanced nursing knowledge and integrates this knowledge and experiential base into clinical practice.
  • Demonstrates competence to practice autonomously in the implementation of strategies to promote health and prevent illness.
  • Proficient in planning, implementing, and evaluating complex care, and demonstrates competency to teach others.
  • Provides for problem solving among staff/between staff, patients, residents’/clients and with physicians and multidisciplinary team.
  • Applies skills in consultation, collaboration, and systems management in providing effective care.
  • Works in collaboration with the Shawano pharmacy team, services provider(s), home and community care programs, and nursing station staff to facilitate the Outpatient Dispensing Program.
  • Willing and capable to be on an on-call basis to facilitate patient care service(s) based on current Clinical Practice Guidelines and the FNIHB and /or SERDC Formulary.
  • Counsel client(s) on symptom management, health maintenance, pharmacotherapy, alternative therapies, rehabilitation strategies and other health programs.
  • Communicate with clients about health assessment findings and diagnoses, further required testing and referral to other health-care professionals; they are also responsible for client follow-up.
  • Provide leadership in the development, implementation and evaluation of strategies to promote health and prevent illness and injury, work with inter-professional teams, other health-care providers/sectors and community members.
  • Maintains appropriate file systems and records with guides and indexes, including computer records, back-up files, etc.
  • Maintain/renew professional license according to expectations of the College of Registered Nurses of Manitoba.
  • Ensure deadlines/timelines are met in a timely manner, this includes preparation of reports for funder(s) and liaise with referring sources as requested and required.
  • Maintain program statistics for purposes of evaluation and research.
  • Participate in internal/external meetings as required and identified by immediate supervisor.
  • Ability to work effectively as a team member - work well with others and is confident in all aspects.
  • Adhering to all SERDC policies.
  • During an emergency epidemic/pandemic situation, job duties will be reviewed and reassigned accordingly.
  • All other related duties assigned by your supervisor and the Leadership of SERDC.

Qualifications

  • Master's Degree in Nursing. Registered Nurse with current CRNM registration plus training in Primary Care Skills course or equivalent is required.
  • Current registration, in good standing with the College of Registered Nurses of Manitoba (CRNM) Extended Practice Registry is required.
  • Abides by the Canadian Nurses Association Code of Ethics for Registered Nurses.
  • Prescribing Narcotics and Controlled Substances Certification.
  • 2-3 years clinical nursing experience, preferably in a Northern clinic or related health care setting is required.
  • Minimum of 2 years experience as a Nurse Practitioner, preferably in a Northern Clinic or related Health Care setting is required.
  • An acceptable equivalent combination of education and experience may be considered/determined by SERDC.
  • Preference will be given to applicants competent in an Indigenous language, familiarity with the environment of SERDC communities - culture/values/traditions and living conditions is required.
  • Demonstrated effective communication skills in verbal and written forms, skillful to foster inter-personal relationships including effective negotiation, conflict resolution, and problem-solving, decision-making, and group facilitation skills.
  • Proficient with computers, software programs, and applications such as MS Office, telehealth, EMR, etc.
  • Competency in managing technology related to health and healthcare systems.
  • Strong leadership, critical thinking, and change management skillset.
  • Capable to adapt quickly to changing situations with the ability to function in a demanding and stressful environment.
  • Adheres to the scope of practice as documented in the Regulated Health Professions Act (RHPA).
  • Knowledgeable of Regional/Facility/Unit policies, procedures, protocols, and guidelines.
  • Knowledgeable of legislation: Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices (Universal Precautions) as well as other pertinent legislated acts.

Employment conditions

  • Standard 40-hour work week.
  • Additional hours beyond the standard regular hours of work may be required at times.
  • Any work conducted after regular standard working hours will be banked at straight time. Banked time will be taken off in lieu of.
  • Travel - by all forms of transportation, to remote and northern First Nation communities in MB.
  • Current and valid MB Driver’s License with accessibility to a reliable insured vehicle.
  • May be exposed to high crime areas within the service area community.
  • Exposure to weather and temperature extremes.
  • Exposure to indoor office fluorescent lights.
  • Exposure to computer monitors throughout the course of the day.
  • Willingness and ability to work in a self-paced environment while maintaining strong, positive relationships with other members of the SERDC organization
