We are searching for a Program Specialist to join the team! This Program Specialist will work across cross-functional internal teams to manage day-to-day operations of the Kidney Care Management (KCM) program that currently serves >3,000 patients in >800 Aledade practices.

Essential Duties & Responsibilities:

  • Lead day-to-day project management (e.g., own meeting facilitation with internal cross-functional workstreams and external partners, maintain and iterate on existing program management materials, day-to-day relationship management with vendor partner, etc.) while looking for opportunities to improve existing materials and workflows to support the KCM program
  • Lead regular data analysis and maintenance of Tableau dashboards (in close partnership with the Business Intelligence team) to track key program metrics, measure performance, and catch and address any technical issues
  • Support Practice Adoption, Practice Engagement, and Experience Research & Design (XDR) teams to develop/ iterate on program materials, schedule/ facilitate calls with practices as needed, and capturing and responding to practice and field team feedback
  • Support various pilot projects to increase patient engagement, test improvements to existing operational workflows, and drive better cross-program collaboration across Aledade Plus programs
  • Other duties as assigned

Required Skills/Abilities:

  • Proactive problem-solver with a willingness to learn, ask questions, and operate independently in ambiguity
  • Strong communication and facilitation skills
  • Strong organizational skills with an attention to detail
  • Working knowledge of Microsoft Excel/Google Sheets (i.e., must be comfortable with basic formulas like vlookup, pivot tables, etc.) and Tableau
  • Mission orientation - Demonstrated interest/ excitement around driving impact for patients and practices and value-based care
  • Proven ability to work effectively with cross-functional teams
  • Strong analytical skills/mindset

Education & Experience:

  • Bachelorโ€™s Degree
  • Minimum of 3+ years of work experience
  • Healthcare experience in an operations, project management, or client services/account management capacity (preferred)
